Referrals
Many insurance plans require a referral from your primary care provider to see a specialist. To avoid having to pay out-of-pocket costs, please carefully read your benefit information to determine your insurance company's policy on referrals and specialty care.

It can take up to seven business days to process a request for a referral, unless your situation is an emergency. Please plan accordingly when requesting an authorization from you primary care physician and/or insurance company. Please keep in mind that authorizations for specialty care have time and visit limitations. Additional authorizations may be required if your circumstance changes.

Payments and Insurance
If we are a member of your insurance plan, we will submit a claim on your behalf with payment due according to the appropriate provider contract. A current insurance card must be presented prior to services being rendered if you wish to have your insurance billed. Any office visit or surgical co-pay is due at time of service.

If we are not a member of your insurance network, we will be happy to file a claim with your plan, but please be advised that your visit will likely be treated as an “out of network” visit, which may result in you being responsible for a paying a larger portion of the charges.

Please be aware that the filing of insurance claims does not guarantee payment by your carrier. Frequently, proof of eligibility, covered and non covered services and various exclusions cannot be determined until your carrier processes the claim. Therefore, we do submit insurance claims with the understanding that you are responsible for any non-covered service, co-payments and/or deductibles.

For patients without health insurance, we are happy to extend a 25% cash discount to you for payment at the time of your visit. All fees are due at the time of service.
