Office Policies


Prescriptions and refills are handled during regular office hours. To obtain your refill faster, please call your pharmacist and request that they contact our office directly. A 48-72 hour turnaround is standard. In some cases we may require that you schedule an appointment to see the doctor before your prescription may be refilled.

Requests for prescription refills received after hours will be processed on the next business day, unless it is determined by the physician on call that your request is an emergency.

Cancellation and Late Policy

If you arrive more than 10 minutes late we may have to reschedule your appointment.

A $50.00 charge for no-show appointments or cancellation of appointments without 24-hour advance notice may be assessed.

New Patient Policies

We ask that all new patients arrive at least 15 minutes prior to their appointment time to complete patient information forms.

New patients under the age of 18 must be accompanied by a parent or guardian on their first visit.

Medical Records and Release Forms

We are pleased to comply with all requests to release copies of your medical records. To request your records please contact our office to obtain a Records Release Authorization form.
